If you experience a needlestick or other injury, you and your supervisor must complete an Incident Report, a 1st Report of Injury and call one of FAA's two Worker's Comp carriers.

The completed First Report of Injury Form & Lab Order must go with you when you seek medical treatment. Lab Order forms have the correct
Worker's Comp billing information. If you do not take the forms with you, the facility will possibly bill your insurance/you directly instead of billing FAA's worker's comp insurance.
**A copy of the completed/signed first report of injury and incident report MUST be sent to Inbox: safety@familyallergy.com**
Forms should be forwarded as soon as reasonably possible, but not later than 4 days post injury.
Employees can choose a lab/facility that is close in proximity to our office locations. There is a listing of labs/facilities provided for your convenience.
